Medicare Advantage (Par C) C-SNP

Chronic Special Needs Plan (C-SNP) is a type of Medicare Advantage (Part C) designed for people with chronic conditions.

You can enroll in a Chronic Special Needs Plan (C-SNP) if you’re enrolled in Original

Medicare Parts A and B have diabetes, heart failure or cardiovascular disorder and live in the plan’s service area.

This plan connects you with professionals and services that can meet the health care needs of people with chronic condition.

C-SNP plans usually include routine foot care, routine vision and hearing coverage, and care management services provided by a dedicated team of nurses.

The goal is to give you the support you need to help manage your health and help maintain your independence.

To be eligible to enroll in this plan you must have been diagnosed with at least one of these conditions:

Medicare Advantage (Par C) D-SNP

Dual Special Needs Plan (D-SNP) is a type of Medicare Advantage (Part C) plan designed to meet specific needs for people with full state Medicaid benefits. 

If you receive full State Medicaid benefits the Dual Special Needs Plan may be right for you.

To be eligible for this plan you must be enrolled in Original Medicare Parts A and B, receive full State Medicaid benefits and live in the plan’s service area.
